    MRI features of spinal fluorosis: Results of an endemic community screening

    Objectives: Fluorosis is endemic in many parts of the world. However community studies on MRI features of fluorosis are lacking. The aim of this study was to determine MRI features of spinal changes in a community with endemic fluorosis in the Thar Desert Pakistan.Methodology: Randomly selected adults from the Village Samorindh, district Tharparker, Sindh, Pakistan, with spinal fluorosis diagnosed on plain x-rays and raised serum fluoride levels were studied from June 2008 to January 2009. MRI was carried out on 0.5 T open magnet MRI system. Features of vertebral body, spinal ligaments, intervertebral disc, facet joints, iliac wings and other incidental findings were noted. Sclerosis was defined as low signal intensity on both T1 and T2 weighted images. RESULTS were described as mean and percentage values.Results: All the studied 27 subjects complained of back ache without neurological signs. The average age was 43.33 ± 10.45 years; 21 being male (77.8%). The most frequent findings included generalized vertebral sclerosis (24, 88.8%), ligamentum flavum hypertrophy (23, 85%), anterior (20, 74%) and lateral (17, 62.9%) disc herniation, thickened longitudinal ligaments, and narrowing of spinal foramina. Hemangioma was seen in 04(14.8%). The most commonly involved level was L1-2, L4-5 and lower dorsal spine.Conclusion: Vertebral sclerosis, a combination of premature degeneration with anterior disc herniation and an unusually high frequency of vertebral hemangioma formed the spectrum of MRI findings in subjects with spinal fluorosis having back ache but no neurological findings

    The Nexus between Export Diversification and Foreign Direct Investment: Empirical Evidence from China

    The transformation of the Chinese economy into a high-tech manufacturing hub spurred a heated debate among policymakers and academia. The large-scale market-friendly reforms have resulted in massive FDI inflows and high export diversification along with the extensive as well as intensive margins. By using the ARDL and VECM approaches, this paper revisits the theoretical and empirical connection between export diversification and FDI inflows in the framework of the Melitz model. The cointegration outcomes indicate a long-run relationship between export diversification and FDI, where the later has a diversifying effect on Chinese exports. The estimated coefficients show that export concentration has negatively related to FDI in China. Conversely, the results validate the presence of a positive spillover effect on export diversification. The reverse effect also holds, and the results indicate that export concentration has a negative effect on FDI or in other words, export diversification can attract foreign capital. It means that the diversifying capabilities of firms’ allure FDI inflows. Further, the granger causality analysis confirms the short- and long-run bidirectional causalities between the variables of interest. Overall, the results confirm the validity of the Melitz model and spillover theory. The inclusion of control variables is robust to our analysis. The study advocates several policy implications for the stakeholders

    The asymmetric unit of the title compound, C8H9NOS, contains two independent mol­ecules with the meth­oxy groups oriented in opposite conformations. The mean planes of the carbothio­amide groups are tilted by 7.88 (15) and 11.16 (9)° from the mean planes of the benzene rings. In the crystal, the mol­ecules form dimers via intermolecular N—H⋯S inter­molecular hydrogen bonds, resulting in eight-membered rings of R 2 2(8) graph-set motif. The dimers are further linked by C—H⋯O hydrogen bonds into chains along the c axis. Adjacent chains inter­act through inter­molecular N—H⋯S hydrogen bonds, generating eight-membered rings of R 4 2(8) graph-set motif

    Overhead Based Cluster Scheduling of Mixed Criticality Systems on Multicore Platform

    The cluster-based technique is gaining focus for scheduling tasks of mixed-criticality (MC) real-time multicore systems. In this technique, the cores of the MC system are distributed in groups known as clusters. When all cores are distributed in clusters, the tasks are partitioned into clusters, which are scheduled on the cores within each cluster using a global approach. In this study, a cluster-based technique is adopted for scheduling tasks of real-time mixed-criticality systems (MCS). The Decreasing Criticality Decreasing Utilization with the worst-fit (DCDU-WF) technique is used for partitioning of tasks to clusters, whereas a novel mixed-criticality cluster-based boundary fair (MC-Bfair) scheduling approach is used for scheduling tasks on cores within clusters. The MC-Bfair scheduling algorithm reduces the number context switches and migration of tasks, which minimizes the overhead of mixed-criticality tasks. The migration and context switch overhead time is added at the time of each migration and context switch respectively for a task. In low critical mode, the low mode context switch and migration overhead time is added to task execution time, while the high mode overhead time of migration and context switch is added to the execution time of a task in high critical mode. The results obtained from experiments show the better schedulablity performance of proposed cluster-based technique as compared to cluster-based fixed priority (CB-FP), MC-EKG-VD-1, global and partitioned scheduling techniques e.g., for target utilization U=0.6, the proposed technique schedule 66.7% task sets while MC-EKG-VD-1, CB-FP, partitioned and global techniques schedule 50%, 33.3%, 16.7% and 0% task sets respectively

    Performance analysis of sphere packed aided differential space-time spreading with iterative source-channel detection

    The introduction of 5G with excessively high speeds and ever-advancing cellular device capabilities has increased the demand for high data rate wireless multimedia communication. Data compression, transmission robustness and error resilience are introduced to meet the increased demands of high data rates of today. An innovative approach is to come up with a unique setup of source bit codes (SBCs) that ensure the convergence and joint source-channel coding (JSCC) correspondingly results in lower bit error ratio (BER). The soft-bit assisted source and channel codes are optimized jointly for optimum convergence. Source bit codes assisted by iterative detection are used with a rate-1 precoder for performance evaluation of the above mentioned scheme of transmitting sata-partitioned (DP) H.264/AVC frames from source through a narrowband correlated Rayleigh fading channel. A novel approach of using sphere packing (SP) modulation aided differential space time spreading (DSTS) in combination with SBC is designed for the video transmission to cope with channel fading. Furthermore, the effects of SBC with different hamming distances d(H,min) but similar coding rates is explored on objective video quality such as peak signal to noise ratio (PSNR) and also the overall bit error ratio (BER). EXtrinsic Information Transfer Charts (EXIT) are used for analysis of the convergence behavior of SBC and its iterative scheme. Specifically, the experiments exhibit that the proposed scheme of error protection of SBC d(H,min) = 6 outperforms the SBCs having same code rate, but with d(H,min) = 3 by 3 dB with PSNR degradation of 1 dB. Furthermore, simulation results show that a gain of 27 dB Eb/N0 is achieved with SBC having code rate 1/3 compared to the benchmark Rate-1 SBC codes.Web of Science2116art. no. 546

    Efficacy of the muscle energy technique versus the strain-counter strain technique on immediate deactivation of myofascial trigger points in upper trapezius muscle

    Aims and objectives: Thisstudy aimed to check the comparative efficacy of the Muscle EnergyTechnique versus the Strain-Counter Strain technique on immediate deactivation ofmyofascial trigger points in the upper trapezius muscle. Study design: The study comprised acomparative analytical design to compare and contrast the two study interventions. Place andduration of the study: The research was conducted in the Department of Physical therapy,Allied hospital Faisalabad for 6 months. Patients and methods: Based on inclusion andexclusion criteria, 40 subjects were enrolled in the study using the convenient samplingmethod. The subjects were divided into two groups; group A (n=20) received a single sessionof baseline treatment with themuscle energy technique, while group B (n=20)received a singlesession of baseline treatment with the strain-counter strain technique. The subjects wereevaluated through the pressure-pain threshold (algometer), Numeric Pain Rating Scale, andModified Bournemouth Questionnaire as pre-intervention and post-intervention measuringtools for pain and functional status. Results: The data was analyzed using SPSS version 17.Within group analysis showed a significant difference between pre- values and post values ofpressure-pain threshold, Numeric Pain Rating Scale and Modified BournemouthQuestionnaire in both groups (P<0.05). Between group analysis was done using independentsample t test. It also showed significant difference (P<0.05) in post mean values between thetwo group subjects in all three outcomes. The post-mean values for the strain-counter straingroup were slightly more improved than the Muscle Energy Technique group. Conclusion:The strain-counter strain technique is found more effective than the Muscle Energy Techniquefor immediate deactivation of myofascial trigger points in the upper trapezius muscle

    A survey on computational intelligence approaches for predictive modeling in prostate cancer

    Predictive modeling in medicine involves the development of computational models which are capable of analysing large amounts of data in order to predict healthcare outcomes for individual patients. Computational intelligence approaches are suitable when the data to be modelled are too complex forconventional statistical techniques to process quickly and eciently. These advanced approaches are based on mathematical models that have been especially developed for dealing with the uncertainty and imprecision which is typically found in clinical and biological datasets. This paper provides a survey of recent work on computational intelligence approaches that have been applied to prostate cancer predictive modeling, and considers the challenges which need to be addressed. In particular, the paper considers a broad definition of computational intelligence which includes evolutionary algorithms (also known asmetaheuristic optimisation, nature inspired optimisation algorithms), Artificial Neural Networks, Deep Learning, Fuzzy based approaches, and hybrids of these,as well as Bayesian based approaches, and Markov models. Metaheuristic optimisation approaches, such as the Ant Colony Optimisation, Particle Swarm Optimisation, and Artificial Immune Network have been utilised for optimising the performance of prostate cancer predictive models, and the suitability of these approaches are discussed

    Antimicrobial resistance among migrants in Europe: a systematic review and meta-analysis

    BACKGROUND: Rates of antimicrobial resistance (AMR) are rising globally and there is concern that increased migration is contributing to the burden of antibiotic resistance in Europe. However, the effect of migration on the burden of AMR in Europe has not yet been comprehensively examined. Therefore, we did a systematic review and meta-analysis to identify and synthesise data for AMR carriage or infection in migrants to Europe to examine differences in patterns of AMR across migrant groups and in different settings. METHODS: For this systematic review and meta-analysis, we searched MEDLINE, Embase, PubMed, and Scopus with no language restrictions from Jan 1, 2000, to Jan 18, 2017, for primary data from observational studies reporting antibacterial resistance in common bacterial pathogens among migrants to 21 European Union-15 and European Economic Area countries. To be eligible for inclusion, studies had to report data on carriage or infection with laboratory-confirmed antibiotic-resistant organisms in migrant populations. We extracted data from eligible studies and assessed quality using piloted, standardised forms. We did not examine drug resistance in tuberculosis and excluded articles solely reporting on this parameter. We also excluded articles in which migrant status was determined by ethnicity, country of birth of participants' parents, or was not defined, and articles in which data were not disaggregated by migrant status. Outcomes were carriage of or infection with antibiotic-resistant organisms. We used random-effects models to calculate the pooled prevalence of each outcome. The study protocol is registered with PROSPERO, number CRD42016043681. FINDINGS: We identified 2274 articles, of which 23 observational studies reporting on antibiotic resistance in 2319 migrants were included. The pooled prevalence of any AMR carriage or AMR infection in migrants was 25·4% (95% CI 19·1-31·8; I2 =98%), including meticillin-resistant Staphylococcus aureus (7·8%, 4·8-10·7; I2 =92%) and antibiotic-resistant Gram-negative bacteria (27·2%, 17·6-36·8; I2 =94%). The pooled prevalence of any AMR carriage or infection was higher in refugees and asylum seekers (33·0%, 18·3-47·6; I2 =98%) than in other migrant groups (6·6%, 1·8-11·3; I2 =92%). The pooled prevalence of antibiotic-resistant organisms was slightly higher in high-migrant community settings (33·1%, 11·1-55·1; I2 =96%) than in migrants in hospitals (24·3%, 16·1-32·6; I2 =98%). We did not find evidence of high rates of transmission of AMR from migrant to host populations. INTERPRETATION: Migrants are exposed to conditions favouring the emergence of drug resistance during transit and in host countries in Europe. Increased antibiotic resistance among refugees and asylum seekers and in high-migrant community settings (such as refugee camps and detention facilities) highlights the need for improved living conditions, access to health care, and initiatives to facilitate detection of and appropriate high-quality treatment for antibiotic-resistant infections during transit and in host countries. Protocols for the prevention and control of infection and for antibiotic surveillance need to be integrated in all aspects of health care, which should be accessible for all migrant groups, and should target determinants of AMR before, during, and after migration.     The global burden of adolescent and young adult cancer in 2019 : a systematic analysis for the Global Burden of Disease Study 2019

    Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world. With notable differences observed across SDI settings, these estimates can inform global and country-level cancer control efforts. Copyright (C) 2021 The Author(s).
